Mejo collects your user name, email address and telephone number from you directly, and free form fields present in the Online Services allow you to input information of your own choosing, which may include but is not limited to date of birth, present age, gender, blood type, height, weight, medical diagnoses, medical conditions, medications, medical devices, medical support needs, medical care history including surgery and doctor visit information, therapies, diet, immunizations, emergency contacts, medical care personnel, school or caregiver information including contact information, insurance providers, and other information (inclusively “Personal Information”).
